Session 1 : Current Concepts in Management of Hospital Acquired Pneumonia by Dr. Chiche Jean Daniel, Full Professor of Critical Care Medicine at Paris Descartes University
Classification: HAP is categorized into non-ventilated HAP (NV-HAP), ventilator-associated pneumonia (VAP), and ventilator-associated tracheobronchitis (VAT), reflecting different risks and outcomes.
Diagnosis: Clinical signs are nonspecific. Lower respiratory tract sampling improves accuracy. Europe favors invasive distal sampling; the US accepts non-invasive methods.
Microbiology & Empiric Therapy: Early-onset often involves community bacteria; late-onset usually MDR organisms. Therapy depends on MDR/mortality risk—ranging from monotherapy to broad-spectrum combinations.
Treatment & Outcomes: Guideline-compliant empiric therapy improves survival. Newer drugs (e.g., ceftolozane-tazobactam) show higher success in MDR infections. Mortality remains high.
Immunocompromised Patients: Broader pathogen spectrum; diagnosis often requires invasive sampling, PCR, and biomarkers for targeted therapy.
Prevention & Stewardship: VAP prevention bundles (oral care, sedation breaks, hygiene) are key. Biomarkers (e.g., procalcitonin) help shorten antibiotic duration, supporting resistance control.

High Burden: Tropical fevers like dengue, malaria, and scrub typhus cause major public health challenges in India.
Overlapping Symptoms: Similar presentations (fever, rash, MODS) make differential diagnosis difficult.
Severe Complications: Risk of shock, hemorrhage, organ failure, and high mortality without timely care.
Early Empiric Management: Urgent supportive care and empiric antimicrobials are critical before test results.
Diagnostics & Prognosis: RDTs/serology aid diagnosis; factors like secondary dengue or malaria immunity predict severity.
Syndromic Approach: Combine rapid tests, empiric antibiotics (e.g., doxycycline, ceftriaxone), and organ support to reduce mortality.
Session 3 : Panel Discussion - Principle and Implementation of Antimicrobial Stewardship Program. By Moderator Dr. Kapil Borawake and Panelists Dr. Rahul Pandit, Dr. Rajesh Mishra, Dr. Sunil Karanth, Dr. Saswati Sinha, Dr. Deepak Jeswani.
This expert panel discussion comprehensively addressed the multifaceted challenges of sepsis management and antibiotic stewardship in the Indian context, focusing on the practical hurdles of implementing stewardship programs at various administrative levels, debating the most rational early detection tools like SIRS and qSOFA for resource-limited settings, and emphasizing the critical role of diagnostics including rapid tests and biomarkers like procalcitonin, BDG, and galactomannan to guide empirical and targeted therapy for bacterial and fungal sepsis, while also strategizing on antibiotic choices for complex scenarios involving multi-drug resistant Gram-negative and Gram-positive infections.
