Early Detection is Critical: -Delays in diagnosing sepsis, especially in elderly or immunocompromised patients, can worsen outcomes. Traditional biomarkers (e.g., lactate, CRP) may rise too late.
Subtle Clinical Clues: -Altered Mental Status (AMS): New confusion or agitation in elderly patients, even without fever, can signal sepsis due to neuroinflammation.
-Tachypnea: Early rapid breathing often precedes hypotension or hypoxia, triggered by cytokines, lactic acidosis, or microvascular lung injury.
-Microcirculatory Signs: Skin mottling, cool extremities, and delayed capillary refill (>2 sec) indicate poor perfusion despite normal blood pressure.
-New Oxygen Demand: Hypoxia may arise from endothelial dysfunction and microthrombi before ARDS develops.
Pathophysiology:
-Cytokines (e.g., TNF-α, IL-6) disrupt the blood-brain barrier and vascular function.
-Microvascular failure leads to tissue hypoxia and organ dysfunction.
Advanced Tools:
-AI Models: Algorithms like TREWS predict sepsis 4–12 hours before clinical signs.
-Biomarkers: Angiopoietin-2 and Syndecan-1 detect endothelial injury early.
-Host Gene Signatures: Immune response profiling stratifies patients by sepsis risk and guides personalized treatment.
Future Directions:
-Wearable tech for continuous monitoring (e.g., HR variability) and precision medicine approaches (e.g., immunostimulants for immune-paralyzed patients).
